How do I know if I am eligible ?

 
 

You must fulfill all of the following criteria:

  • Be age 60 or older
  • Meet the Medicaid income and asset limits
  • Require the level of care of a nursing home
  • Choose community-based services rather than a nursing home

How can the PDA Waiver Program help me?
 
 
After a thorough assessment to determine your eligibility, a Care Manager will design a care plan with you and will monitor the appropriate services.  If you are eligible, the following services may be available at no cost through the PDA Waiver Program to help you live more comfortably at home:
 
 
  • Personal Assistance
  • Personal Care
  • Respite Care
  • Home Support
  • Companion Services
  • Home Delivered Meals
  • Older Adult Daily Living Centers
  • Specialized Medical Equipment and Supplies
  • Personal Emergency Response System
  • Environmental Modifications
  • Transportation

Who would actually provide these services to me?
 
 

If you participate in the PDA Waiver Program, a Care Manager will be assigned to you.  After you and your Care Manager agree on a plan to meet your needs, the Care Manager will authorize the services.  You will then be able to choose which company/agency you want to provide the services.  You may select from a list of participating providers.  All service providers are certified by the Pennsylvania Department of Public Welfare to ensure that they meet Medicaid standards.   

 
     
 
Are there limits to the extent of services that I can receive at home?
 
 

While there are no specific limits to the number of services you may receive, there are limits to the amount of services.  With limited exceptions, the cost of your care plan cannot exceed a percentage of the cost of Pennsylvania's average Medicaid nursing home daily charge.
