What are Nursing Home Transition Services (NHTS)?

Nursing Home Transition Services identifies nursing home residents wanting to move from the nursing home to the community.  The Nursing Home Coordinator will assist in the transition to home.  This includes making arrangements for housing and services at home.  Nursing Home Transition Services are a coordinated effort of the Governor’s Office of Long Term Living, the Pennsylvania Departments of Aging and Welfare, Blair Senior Services, Inc., and Community Resources for Independence, Inc.

Nursing Home Transition Services are available to nursing facility residents who desire to move back into the community.  Once the move from the nursing facility to the community has been completed, individuals may receive services through one of Pennsylvania’s Waiver Programs, or other Aging Home and Community Based Services.

The Nursing Home Transition Coordinator will help you …

• interact with nursing home personnel concerning the transition.
• locate housing suitable to meet your needs
• obtain household items such as furniture, bedding and kitchenware
• obtain funding for required security or utility deposits
• obtain assistance for moving or setting up the household
• understand the ongoing care available to you through a Home and Community Based Service program.

Anyone can make a referral, including the resident, a family member, friend, or nursing home employee.

Blair Senior Services’ Nursing Home Transition Coordinator, along with the resident, caregivers, physician and nursing home staff, will develop a plan to relocate the resident.
Based on medical and financial eligibility, home and community based services may include:

• Home Delivered Meals
• Nursing
• Companion Care
• Medical Equipment and Supplies
• Personal Emergency Response System
• Home Support
• Environmental Modifications
