In order to use Blair Senior Services’ Medical Assistance Transportation Program, the following requirements must be met:
Applicant must be eligible for Medical Assistance and display a current MA card;

Applicant must be a permanent or temporary resident of Blair County;

Applicant must complete and sign an eligibility form for program registration, and must recertify periodically at the request of the MATP provider; and,

Applicant must have a need for transportation assistance to receive MA service.
MA transportation may be provided through one of several means of service. The MA provider will evaluate each request and determine the most appropriate type of transportation service based on cost efficiency and client need.

Urgent Care Requests |

Any illness or severe condition which under reasonable standards of medical practice would be diagnosed and treated within a twenty-four (24) hour period and, if left untreated, could rapidly become a crisis; or discharge from a hospital will be delayed until services are approved; or member’s/consumer’s ability to avoid hospitalization depends upon prompt approval of services.

Additional information concerning the MATP requirements: |

- MATP will not fund trips of less than ¼ mile unless the consumer is unable to walk that distance.
- MATP, in all cases, shall utilize the most cost-efficient service available that meets the consumer’s need for service.
- MATP does not provide emergency ambulance transportation.
- MATP does not provide litter (stretcher), door-through-door, or attendant required transportation.
- MATP does not provide transportation to emergency rooms, sheltered workshops, day care programs, WIC programs, VA medical centers, or transportation to non-medical services or for visitation purposes.
IF THE REQUESTED SERVICE IS UNAVAILABLE THROUGH THE MEDICAL ASSISTANCE TRANSPORTATION PROGRAM, CONSUMERS WILL BE REFERRED TO THE BLAIR COUNTY ASSISTANCE OFFICE.
All eligible consumers will be required to accept the most cost-efficient form of transportation that meets their needs. Any consumer who refuses such transportation will then be referred to the Blair County Assistance Office. A refusal of service by the consumer is not a denial, termination, or reduction by the MATP Program. Therefore, a written notice will not be issued.

Under the provisions set forth by the Department of Public Welfare for Medical Assistance Transportation Program providers, Blair Senior Services, Inc. has the right to sanction consumers who have failed to comply with the Agency’s program instructions, rules and requirements. Your Right To Appeal and Have a Fair Hearing: |

Any MA eligible consumer whose request for service has been denied, reduced, or terminated, will receive written notification and information on their right to request a fair hearing from the Department of Public Welfare.
